STEP 1: Fill in the Purchase Form.(You can put specific issues or description of the problem in your Notes section) We offer Rush Processing on your order that is $5 more. The rush processing fee does not come off of your total price.

STEP 2: Print and Fill in this form.

STEP 3: Upon recieving the payment confirmation e-mail via PayPal mail the form and the iPod to the following address. Mail only the iPod (no chargers needed), and the repair form.
